If you miss your appointment or need to reschedule within 24 hours of your scheduled appointment, you will be charged a $100 service fee. This fee must be paid before you are able to schedule your next appointment. The service provider reserves their time and the treatment room for you. This fee is charged regardless of the circumstances, including cancelling due to Covid. By scheduling an appointment, you are agreeing to our rescheduling/cancellation policy.
Patients with two or more consecutive cancellations or reschedules with less than 24 hours of notice will be expected to prepay a deposit of $100 at time of scheduling. This fee will be applied to the scheduled appointment at check out.
A consultation fee of $100 will be charged at time of scheduling your appointment with Dr. Lichtenberger. This fee will be applied towards your injection, treatment, or surgery. You will forfeit the fee, if you no show, cancel or reschedule your appointment with less than 24 hours’ notice.
Patients who have previously consulted on a treatment or surgery plan but would like further consultation with their provider may be subject to an additional $50 fee for each additional 30-minute consultation. This fee will go towards the service should you continue forward with us.
Clients and patients will be required to complete intake forms for your first visit and will be asked to update this information yearly. These forms are an important part of your treatment or service with us to ensure that we have a clear picture of your medical history, medications, the reason for your visit and contact information. Consent forms must be signed for every treatment appointment. Intake forms will be mailed to you before your first appointment. We ask that you complete the forms and return them to our office upon arrival for your appointment. If you are unable to, please arrive 15 minutes early to complete all intake forms.
LATE ARRIVAL – If arrival is delayed, we will make every effort to accommodate your full appointment, but service time may be abbreviated to avoid delays for other clients and patients. It is up to the provider to decide if there is enough time to see you. You may be asked to reschedule your appointment. Abbreviated treatments are charged at full price.
Refund Policy / Product Returns
Request for refunds will be reviewed on a case-by-case basis. Product refunds may be issued within 30 days of product purchase.
Prices and services are subject to change at any time. Botox Party prices will no longer be available.
Specials, Coupons, Discounts and Event pricing cannot and will not be extended. These are promotions meant to be used in a timely manner.
Once a package is purchased it has no cash value and cannot be exchanged for other services (unless advised by professional) or transferred to another patient.
We do not offer in house financing; however, we do accept Care Credit. You may finance payments with them for 6, 12, and 18 months interest free.
Payment is due in full at the time of treatment.
Point Programs -Alle, Aspire, Evolus
We do offer reward programs through Alle (Botox, Juvederm, SkinMedica and Latisse), Aspire (Dysport, Restylane, and Sculptra), and Evolus (Jeuveau). It is the patient’s responsibility to inform the front desk of their enrollment in these programs. Points will be issued for each qualifying purchase on the date of service. Points may be redeemed at your next visit. We do not keep track of when your points or coupons expire.
